PQS II – Safety OSHA Application

For those with an OSHA 10-hour, OSHA 30-hour or OSHA authorized trainer card you must complete this form in order to receive credit for PQS II — Safety. Payment of $50 is required for administration fees. Following payment, a PQS II shirt, hard had sticker and certificate will be mailed to you.
